Confrontation in Motivational Interviewing: Threat to Therapy

Abstract

The current exploratory case study was interested in how directive is interactionally accomplished during Motivational Interviewing (MI). The research drew upon studies from other psychotherapeutic or institutional settings to identify interactionally accomplished confrontation as a key potential candidate for performing directive in MI. The aims of the current study were to explore an MI narrative for the presence of confrontations and to describe how client and counsellor introduce and manage the production of such confrontations. The research drew upon archival data in the form of audio-visual recordings of real-life MI counselling sessions. Confrontations located in the data were transcribed and a conversation analysis was then performed to explicate the features of the events. The study produced two key findings. Firstly, confrontations were found within the collaborative narratives of MI, occurring across a three-part contradiction sequence. Second, confrontations were delicately managed by the participants to prevent threatening the collaborative partnership upon which MI is built. By identifying and describing one way in which MI is practically accomplished, the findings of the current study have the potential to contribute to the MI literature and the development of future training practices. Additionally, the study examples the potential benefits of performing psychological research with a focus towards the processes applied during practice-in-action as oppose to focusing on outcome measures.
