The Influence of Stoicism on NBCSP Compliance: A Comparison of Geographic Regions

Abstract

Living in a non-metropolitan area in Australia is associated with a higher risk of colorectal cancer (CRC) diagnosis as well as poorer outcomes from the disease. People that live in these communities are often diagnosed when the cancer has already reached an advanced stage and is, as a result, harder to treat. Previous research has shown that older adults living in non-metropolitan areas also have lower rates of participation in the National Bowel Cancer Screening Program (NBCSP) than those living in other areas of Australia. This geographical disparity may be partly explained by attitudinal factors. One trait that has been found to influence the likelihood of health-related help-seeking is stoicism. The aim of this research is to investigate whether the trait of stoicism, as measured by Mansfield’s (2005) two sub-scales Need for Control and Self-Reliance (NFCSR) and Minimising Problems and Resignation (MPR), can predict an individual’s compliance with the NBCSP and whether this effect varies by geographic location. Data was collected through a cross-sectional survey of adults aged over 50 in Australia measuring stoicism and adherence to NBCSP. It was hypothesised that stoicism would be negatively associated with NBCSP compliance and that this effect would be stronger in non-metropolitan areas versus metropolitan areas. The results revealed that only the MPR sub-scale significantly negatively influences NBCSP compliance, particularly in non-metropolitan areas. That is, the tendency to minimise problems until they become serious was more likely to lead to non-compliance in rural and remote areas. Strategies to increase compliance should focus on this aspect of stoicism by normalising preventative help-seeking behaviours such as early screening in the absence of symptoms or the presence of minor symptoms.
